The Application forms listed must be used to make an application. Applications submitted on alternative documents or in other formats may not be accepted.

Please ensure:

  • The application form is fully complete.
  • Ensure you have supplied Referee(s) and that your Referee(s) are aware you have provided their information as we may contact them to confirm details.  You must have known your Referee(s) for at least 2 years.  Referee(s) must be resident in Great Britain, not be a member of your family, a registered firearms dealer, a serving police officer, police employee or police and crime commissioner or their support staff.  Referee(s) must be of good character.
  • Include passport photograph. Please also print your name on the reverse of the photo. 

Please send the application form, photo, Referee(s) form with a cheque or postal order (made payable to West Mercia PCC) to:
