  7. Please provide us with the crime reference number in the box at the end of the form and you will be exempt from paying the £4 fee.
  8. If your certificate has been damaged you may be exempt from paying the £4 fee if you are able to provide us with any remnants. Please send in a covering letter with the remnants, but if no remnants are available please make a cheque for £4 made payable to: West Mercia PCC.
