You are about to start the process of making a complaint. We take this very seriously, so please provide as much information as you can to help us with our investigations.

We are using a national complaints form so you'll need to:

  1. download the form here
  2. complete and save it
  3. attach the form to an email and send it to the Professional Standards Department
