I write in connection with your request for information which was received on 23rd June 2025. Please find below the response to your request:
The 2 police officers had attend a domestic incident in East London at 05.04 on 18/2/2023. Due to miscommunication neither police officer completed a DASH (Domestic Abuse, Stalking and Harassment) risk assessment. PC Thornton completed the DASH on the force computer system without having asked the victim the questions on the DASH. The outcome of the disciplinary hearing was PC Thornton was given a final written warning and PC Finch a written warning.
Q1. Have any police officer in you force been disciplined since 1/1/2013 for either failing to complete a DASH and/or falsifying a DASH.
